Athletic clearance for the 22-23 school year is now open. Students that will be participating in sports must complete the clearance process. This must be done each year. For those who have done this in previous years you will notice the site looks different…it has been upgraded. Below is the link to the site along with some helpful information links. Go to California, school is Wiseburn Da Vinci, year is 22-23. You can download the physical form when you get to the medical history part. On the file upload page you will need to upload both the physical and proof of insurance.
